The planner is divided into sections that included places to keep information like allergies, immunizations, medications, family history, surgical history, and lifestyle history. There is a calendar to record all appointments and it lists questions that everyone should ask themselves before they go into an appointment, and ask their doctor while they are there. It also leaves space for the patient to write down questions he or she may have for their doctor, but would normally forget if they didn’t have them on paper. The planner also has space to list emergency contact information, lab and x-ray tests ordered, and healthcare provider’s information.
