i2i Systems today announced it has been selected by Tennessee Primary Care Association to provide Population Health Management and Analytics for several of the state’s Federally Qualified Health Centers.

Fourteen FQHCs, representing 130 clinics throughout Tennessee, will implement i2i System’s tools to improve their capacity to generate reports that can be used to inform quality improvement efforts, increase understanding of financial and operational status and meet funder requirements.

“One of the greatest challenges our health centers have had is getting data from their electronic health record systems to assist them with their reporting requirements,” said Kathy Wood-Dobbins, CEO of Tennessee Primary Care Association.

“We are in the midst of a major transformation in health care which includes significant payment reform. Access to quality and performance data is critical to health centers operating in this new environment. Strategic use of health center data supports efforts to achieve Patient Center Medical home recognition and other payment-drive quality initiatives. i2i Systems makes it easy to access the data we need to weather this transition,” she continued.

i2i Systems offers the most advanced, comprehensive Population Health Management and Analytics Solutions available.

i2iTracks is the flagship Population Health Management solution that empowers front-line care teams to proactively care for patients. It provides powerful analytics at the patient, provider and site level by unlocking and standardizing data from disparate electronic health records, practice management and other systems, simplifying the path to actionable information for an entire patient population.

PopIQ, is a cloud-based system that aggregates de-identified patient data across multiple organizations and allows for comparative analysis. It also allows users to generate reports and dashboards across a virtually unlimited number of measures and indicators.

“We are proud to support organizations like TPCA who are striving to build sustainable health centers that deliver the highest quality proactive care possible,” said Janice Nicholson, CEO of i2i Systems.
